Not sure if they are for paid members only, but you have to pay a fee to get a rename code.

The FAQ (http://www.livejournal.com/support/faqbrowse.bml?faqid=25&q=rename+token&lang=) says, "You can rename any personal or community journal to another username as long as the other username is unregistered; deleted and purged; or a personal journal under your control." =] It's $15.
